If you receive a call on your GSM (Vodacom, Telkom Mobile, Cell-C, MTN Number) while being on an active call via your Othos Mobile app, the app will put the active call on-hold and give preference to the GSM inbound call.
If that is NOT the behaviour you want, you may change that - making the GSM call ring as a second call:
Settings > Preferences > Controls > "On GSM call" = "Do nothing"
Settings > Preferences > Controls > "Handle GSM call when" = Answered
